Sequestração Pulmonar: um diagnóstico a não esquecer

RESUMO

A sequestração pulmonar é um diagnóstico frequentemente esquecido, quer pela sua raridade, quer pelas múltiplas formas de apresentação. Pode mimetizar outras doenças pulmonares, sendo passível de vários diagnósticos diferenciais. Os autores descrevem dois casos clínicos de sequestração pulmonar intralobar com diferentes formas de apresentação: um dos casos como achado radiológico, no outro caso o diagnóstico foi feito na sequencia de um quadro pneumónico agudo. Ambas as situações projectavam-se na telerradiografia do tórax junto à goteira paravertebral esquerda, comprometendo o segmento posterior do lobo inferior esquerdo, localização mais frequentemente descrita na literatura. A T.A.C. torácica e a aortografia permitiram demonstrar a existência de vascularização anómala, conduzindo ao diagnóstico definitivo e a uma correcta abordagem cirúrgica. Num dos casos observaram-se aspectos histológicos sugestivos de infecção tuberculosa intra-sequestração o que constitue uma associação de extrema raridade. É feita complementar-meote uma revisão da sequestração pulmonar intra e extralobar.

SUMMARY

Pulmonary sequestration is a rare disease with multiple clinical presentations. Diagnosis is often delayed because this condition may be confounded with other pulmonary diseases. The authors report two cases of intralobar sequestration: in one of them the patient was assyntomatic and diagnosis was based on radiographic anomaly. In the other case patient complained of an acute respiratory illness. Chest X-ray shadows were seen near the left paravertebral gutter in both cases. Computed tomographic scan and angiography confirmed the presence of anomalous vascularization. In one of the cases histological findings were suggestive of tuberculous intra-sequestration infection, which is a very rare association. A revision of intra and extra lobar sequestration is also made.
