Rigor das taxas portuguesas de mortalidade por asma em 1991

RESUMO

O estudo das taxas de mortalidade é importante na caracterização da evolução clínica e na avaliação das terapêuticas utilizadas nas diversas patologias. Recentemente, publicámos as taxas de mortalidade portuguesas referentes à asma brônquica. Com o objectivo de as avaliar propusemo-nos verificar o seu rigor em 1991. Com a colaboração do Instituto Nacional de Estatística (INE) identificámos os verbetes que nesse ano foram codificados como 493.9 (A) e os que, com outra codificação apresentavam a palavra "asma", "asmático" ou "asmatiforme" na causa de morte, causa antecedente ou outros estados mórbidos (NA). Com base nos dados obtidos no INE e recorrendo às diversas conservatórias do país soubemos a identificação dos óbitos e dos médicos responsáveis pelos certificados. Elaborámos inquéritos para serem preenchidos pela família e pelo médico. Adicionalmente obtivemos relatórios de internamento e de autópsias. Identificámos 392 (A) e 79 (NA). Obtivemos informação relativa à história clínica e/ou à morte em 212 (A) (54%) e em 57 (NA) (72%). Usando critérios publicados dividimos as respostas relativamente a diagnósticos de doença e diagnósticos de morte, em 3 categorias: l:Asma; 2:Não Asma; 3:Indeterminado. Dividimos a amostra em 3 grupos etários: <35 anos; 35-70 e >70. Obtivemos os seguintes resultados percentuais:

Os dados obtidos permitem concluir que as taxas de mortalidade por asma são muito inferiores às publicadas e que a percentagem, observável, de falsos negativos é baixa.

ABSTRACT

The study of mortality rate is important for the characterisation of the clinical evolution and in the evaluation of the applied therapeutics in several pathologies. We recently published the Portuguese mortality rates relating to asthma. With the aim to validate them, we propose to check his accuracy in 1991. With the collaboration of the Statistic National Institute (INE) we identifed the official forms that in this year were codified as 493,9 (A) and those which with other codification showed the word " asthma" "asthmatic" or "asthmati-form" as cause of death, previous cause or other morbid states (NA). With the data we got from INE and investigating in several conservatories in our country, we knew the identification of the obits and the doctors responsible by the certificates. We made questionnaires to be filled by the family and the doctor. We also got reports of internment and of autopsies. We identified 392 (A) and 79 (NA). We got information related to the clinical history and/or the death in 212 (A) (54%) and in 57 (NA) (72%). Using published criteria we divided the answers relatively to diagnosis of disease and diagnosis of death in 3 groups: 1: Asthma; 2:No Asthma; 3:Unknown. We divided the sample in 3 aged groupes: a: < 35 years; b:35-70 and c:> 70. We got the following percentage results:

The achieved data allow conclude that the mortality rates by asthma are tower than the ones published and that the investigated percentage of untrue negatives is low.
