RESUMO

No presente caso clínico descreve-se um doente com poliserosite, trombocitopénia autoimune e artrite não erosiva. Apresenta ainda lesões cutâneas sugestivas, mas não suficientemente expressivas para a sua inclusão nos critérios de Lupus Eritematoso Sistémico (LES) definidos pela Associação Americana de Reumatologia (ARA) em 1982.

Assim, apesar deste doente ser ANA negativo e apresentar apenas 3 dos critérios definidos pela ARA os autores sugerem a hipótese de diagnóstico de LES devido às características das lesões cutâneas observadas e à positividade do “lupus band test”.

Os autores aproveitam para fazer uma breve revisão sobre o “lupus band test” e sua elevada sensibilidade e especificidade no diagnóstico de LES.

ABSTRACT

In the following case report is presented a patient with polyserositis, autoimmune thrombocithopenia and arthritis. He presents also skin lesions that suggest Systemic lupus erithematosus (SLE), but that were not considered typical enough for the inclusion in the criteria defined by the American Rheumatology Association (ARA) in 1982.

Although the ANA was negative and only 3 of the ARA criteria are fulfilled, the authors made a presumptive diagnosis of SLE supported by the skin lesions and a positive lupus band test.

The high sensibility and specificity of the lupus band test in the diagnosis of SLE is briefly revised.
