RESUMO

A beta 2 microglobulina é uma proteína encontrada em todas as membranas de células nucleadas, particularme nte os linfócitos.

Procurou-se determinar a importância da dosagem de beta 2 microglobulina no diagnóstico diferencial da síndroma de derrame pleural; definir a importância de dosagem da beta 2 microglobulina na investigação de síndromas de derrames pleurais consequentes a tuberculose pleural. Determinar, estatisticamente, o limiar crítico discriminativo com eficiência máxima da beta 2 microglobulina pleural, em derrames pleurais tuberculosos.

Realizou-se um estudo prospectivo com base na dosagem de beta 2 microglobulina, pelo método da quimioluminescência, em 42 líquidos pleurais de pacientes entre 14 e 88 anos de idade, entre Março de 1998 e Março de 1999, do Ambulatório de Pneumologia da Universidade Federal Fluminense.

Trinta e nove líquidos pleurais foram avaliados e a casuística composta por 21 (54%) indivíduos do sexo masculino e 18 (46%) do sexo feminino. Vinte (51%) casos de tuberculose pleural, 5 (13%) derrames pleurais malignos, 4 (10%) embolias pulmonares, 4 (10%) derrames par apneumónicos, 2 (5%) empiemas, 1 (3%) caso de lúpus eritematoso sistémico e 3 (8%) transudatos.

A dosagem pleural de beta 2 microglobulina é útil como auxiliar ao diagnóstico diferencial de derrames pleurais visto apresentar-se em níveis elevados em derrames pleurais com riqueza linfocitária. A utilidade de dosagem da beta 2 microglobulina para o diagnóstico de derrames pleurais de origem tuberculosa também foi comprovada. O limiar crítico discriminativo em derrames pleurais por tuberculose foi de 3 923,0ng/ml com eficiência de 69,23%.

Beta 2 microglobulin is a protein found on all nucleated cell membranes, particularly in lymphocytes.

The aims of this study was determine the importance of the beta 2 microglobulin dosage for differential diagnosis of the pleural effusion syndrome; define the most efficient cutoff and the importance of this dosage for the investigation of pleural effusion syndrome caused by pleural tuberculosis.

An prospective study was accomplished throu- gh dosages of beta 2 microglobulin (chemilumines- cence method) to 42 pleural effusions from patients between 14 and 88 years of age, between March 1998 and March 1999, at the Pleurology Ambulatory of Federal Fluminense University.

Thirty-nine pleural effusions were evaluated and the sample was then composed of 21 males (54%) and 18 females (46%). Twenty (51%) cases of pleural tuberculosis, 5 (13%) malignant effusions, 4 (10%) pulmonary infarction, 4 (10%) parapneumonic effusions, 2 (5%) empyemas, 1 (3%) case of systemic lupus erythematous and 3 (8%) transudates.

Beta 2 microglobulin dosage in pleural effusions is helpful as an auxiliary tool as far as differential diagnosis of pleural effusions once it presents itself in high levels in pleural effusions rich in lymphocytes. There was also proof of the utility of beta 2 microglobulin dosage for pleural effusion diagnosis originated from tuberculosis. The discriminatory cutoff in pleural effusions caused by tuberculosis was of 3 923,0ng/ml with an accuracy of 69,23%.
